FORM B-1008 - Amended Schedules and Statements

If you are amending a schedule, you have the client sign the declaration re: schedule page and submit that along with the amending schedule. Note that it is filed separately.

Hello Listmates,
I am in the process of revising some of my protocols - and I am wondering if
this form is mandatory. I couldn't find any reference to this particular
form in the local rules. I am assuming this form is designed for instances
where you are amending a schedule that doesn't have a space for a signature
- like amending just Schedule B, etc.
However, when you are amending a form such as the means test that contains
a signature line, is this document necessary?
