I still do it and I keep the disclosures in the client's folder.
Never been audited.

It is the practice of my office since BAPCPA took effect, and in keeping
with the paperwork escalation act that shadowed it, to have my clients
sign the disclosures required by 11 USC 527(a) and 342(b)(1). They need to
sign it about 4 times.
No one has ever asked to see it and I now feel like I may be making more
busywork than necessary. How many of our members our still doing this, and
has anyone ever been audited for this documentation by the UST or whomever
may check to see we are making these disclosures? Im considering
discontinuing the effort after a half decade of the charade.
