Does anyone have a fool-proof 3rd party authorization/power of

Chase is arguing with me that my 3rd party authorization form I sent them
is not a sufficient "power of attorney", though it has the name, address,
account no, SSN, and signature of client.
They kept telling me for 2 weeks they don't have my POA on file and I
finally requested a supervisor and she said, "oh yeah, your form is here,
but it's not a POA". I'm currently on hold while she checks to see if she
can accept it.
