Hey Mike,

I just had this issue in a Chapter 13 last month and I was able to get the license back. Just fax them a copy of the Petition page and Scheudle F and you should be okay.

I dealt with a lady by the name of Staci Pugh at 916-255-4060 (fax - 916-255-4016) and we got the license active again. She is the Judgment/Liability Analyst at COntracotrs State License Board.
