Dear Steven,
As far as I know, there isn't a form for rescinding a reaffirmation agreement. Based on my reading of the second paragraph of item 7 of 524(k)(3)(J)(i), the key step is to notify the creditor in writing of the rescission. There doesn't appear to be a requirement that you file the notice with the Court, though it's probably safest to do so.
I have attached one I used a few years ago, which I also filed with the Court. I am giving you the Word version, rather than the PDF, so you can modify it as you see fit. I hope you find it helpful.

Does anyone have a form for rescinding the reaff they'd be willing to
share, or the name or case # of a case they did it. A client has
changed their minds.
