Has anyone ever had to file a Declaration supporting cram down in a

I filed a preconfiramtion memorandum which lays out the arguments, but the
judge wants an affidavit or declaration supporting the cram down of one
non-consenting class.
Does anyone have a sample so I don't have to reinvent the wheel.
