Settled adversary, need to prepare stip re settlement. Template?

Case started out in a 7, which is when the adversary was filed. The case was subsequently converted to a 13, confirmed, but in the meantime, the adversary has been hanging out there. Everyone is finally on the same page and ready to settle. Anyone have a template to go off of, or a case I can look at? Any twist regarding the fact that its now a 13?
