Looking for help on two cases

Good morning,
I need to file a Motion to re-open for the purposes of filing a Motion to
Avoid a Lien that was discharged in a chapter 7 case I filed back in 2012,
does anyone have a sample, template or guidance on this type of motion that
has had success? Also, I am wondering whether a creditor, who's debt was
discharged would have any viable defenses to our motion.
Also, I filed a chapter 7 case back in 2010 and my clients were discharged,
including a 2nd mortgage. This 2nd mortgage lien holder is now threatening
to foreclose and I am wondering whether we can now file a Chapter 13 and
move to have the 2nd lien stripped as the property is still upside down on
the 1st mortgage? Can anyone provide any guidance on this matter?
