Substituting in for pro se debtor

I recently completed a case that was started in pro per. I prepared and filed a Notice of Appearance and it worked fine. I created the association with the debtor on ECF and from that point on I show up on ECF as counsel for Debtor and I receive all ECF notices. The trustee thought I needed to do a substitution but I never did (that makes no sense to me). Case should discharge.

In the code the definition of entity (101(15)) includes person and the
definition of person (101(41)) includes an individual.

It might have to be due to LBR 2091-1 (b)(1)(B), but I don't know what
"entity" means.

Why sub for nobody? I'd usually do a Notice of Appearance.

When subbing in for a pro se debtor is a substitution of attorney form
filed?
If not, what is the procedure for appearing?
Thanks
