Motion to Withdraw as Counsel in chapter 7

I have a chapter 7 case where my retainer limits the scope of my services.l be needed for continued 341 hearing and most significantly for a 2004 examination. The client has not paid the additional funds and I feel I need to withdraw. I could not find anything in the Rutter Guide as to the proper format for such a motion. The Judge is Clarkson.ther authorities for this situation, it would be greatly appreciated.
