Appearance counsel

I know lots of you use appearance counsel for your bankruptcy cases; I'm wondering how you pay them. In other words, is it on a per-case basis, hourly, or per-calendar?
We use someone in NY who charges $x per calendar call regardless of the number of cases handled. Doesn't matter if we have 1 on or 10, she charges a flat rate. She handles work for about a dozen lawyers in SDNY so she's usually one for 4-5 lawyers per calendar call and makes it up in volume.
A friend of mine is doing some appearance work for a small filer and bills hourly. He's looking to pick up more work from a few other lawyers and trying to figure out how others structure their fees.
Feel free to reply off-list if that's easier. Note that I'm not asking about appearance counsel fees, merely structure.
