Getting approved/compensated when hired "in the middle of the 13 case"

Chapter 13 debtor and his lawyer had a falling out. The lawyer's
application to be relieved as counsel was granted.
Debtor, now in proper, comes to me and ask me to convert him to 7.
A. Do I need to apply to be appointed as chapter 13 counsel "in the middle
of the case" to convert to 7
1. if I want to charge debtor $1,000 (compensated from the estate)?
2. if Debtor's brother wants to pay me (not compensated from the estate)?
B. If I am hired "in the middle of the case," can I accept money and put
it in the trust account or do I need to apply for compensation before I can
accept money
1. from Debtor (compensated from the estate)
2. from Debtor's brother on behalf of debtor (not compensated from the
estate)?
