The New Mortgage Declaration Form

The new form for postpetition preconfirmation deed of trust payments
was released, and is attached.
It is now combined with personal property payments, such as for
vehicles.
1. It now asks for the due date as being the FIRST date a payment
is due, not the last date it is due (with grace period). Does that
mean that the Trustees will object if a debtor hasn't made the
payment by the due date?
2. It also no longer seems to ask for any proof of payments to be
attached. Are those no longer necessary (as of June 1)?
3. Does anyone actually file these things for vehicle payments?
