Pre-petition counseling by incarcerated debtor

Hello Listserve members
I'm filing a chapter 7 for a debtor whose spouse, joint debtor, is incarcerated. He is unable to complete the pre petition counseling and believe he qualifies to be exempt from this requirement per 11 USC 109(h)(4), incapacity.
I haven't faced this before and believe that I need to file a motion for a waiver. Would any of you be willing to share a sample or case number.
Any assistance will be greatly appreciated.
