Motion to be Relieved as Counsel of Record

Can I file a motion to be relieved as a counsel of record on an AP? I have
already been subbed out of the main BK case, but I do not wish to remain on
the AP. If so, if someone would be so kind as to help me out with a form, I
would appreciate it.
