When "post" means past, it is attached to the word being modified, as all good prefixes are. Postdischarge.
There is an exception, when it is attached to a proper noun, like post-Obama.
A prefix cannot be attached to a word beginning with a capital letter, so a hyphen is used.
.
So, since discharge is not a proper noun, the proper spelling is postdischarge.
