Lender insistent on reaffirmation agreement

I have an out of state lender who just threatened me with a relief action if
I did not have the reaffirmation agreement for a mobile home loan (over
$40,000 outstanding for a 1976 mobile home). My clients are both elderly
and one has Alzheimers. The statement of intent was to pay as agreed and
the clients have kept their payments current.
I am not inclined to sign the reaffirmation. So tell me, am I being overly
cautious and should I just let it go? Or should I make this Tennessee
lender come here and take me on.
Suggestions?
