Chapter 13 Discharge procedure

I just got a notice in a Ch. 13 (Pre-barf) case called Discharge of Chapter
13 Trustee and Order Closing Case. However, there was no document
issuing the debtor a discharge. The document I received says the case is now
closed. Did something get missed somewhere?

After filing the required certification, I am still awaiting a discharge in
one cases. Although I received court-generated notices that the certification
and application for discharge had been filed (in October 2008), the
discharge has yet to be entered.
