If the fixes you have been sent so far work for you, more power to you.
I've never been able to make pdf convert smoothly into word and I used to
work on Word documents for a living 8 hours a day in the 90's. The problem
comes with the fact that you have several sections in the order because the
format for the proof of service and for the notice of entry is different
(e.g., has little boxes along the top). One easy cheat is to download one
of the orders already provided by the Court in Word as part its local forms
(for example, an order resolving a relief from stay motion), and changing
that around to match your specific order.

Open the PDF. Hit select all and then copy. Then paste into your Word document. Works like a charm, at least in the Mac O/S, and work on the PC side equally as well
