LOU Orders

Okay, I feel silly asking for this, but I'm dysfunctional when it comes to
Microsoft Word and the court's request for Lodge Orders [formatting issues]
I simply cannot get the formatting correct. Would anyone be willing to send
me a generic motion Lodge Order with formatting in tact, that I may amend to
suit my motion and the court's requirements.
My LOU was rejected. :-(
Thanks
