Go to NCLC.org and buy their book, Consumer Law Pleadings. You'll get
templates there too.
Also, just ask for a few case numbers from the Wise Ones and you'll get
plenty of useful templates.
Our Yahoo site has some helpful information and goodies to work with.
I hope this helps.
Also, check the LBR forms as they provide a general format to work with too.
