Reconversion from Chapter 13 to Chapter 7

I have a debtor who filed in pro per a chp 7 which I then converted to a chp 13 because he needed to catch up on his mortgage. The plan was confirmed last year but now he has a loan modification which took care of the arrearages and now there is no need for the chp 13. What is the standard for reconverting back to chp 7. Does any one have a form for this?
