How do I file a complaint against a Chapter 7 trustee?

I think it is our obligation to report Trustee's who don't do their job properly. If enough people complain someone may listen. Doing nothing is the surefire way to guarantee this person lifetime employment at the public's expense.

Through the US T's office. You can either draft a letter stating all the problems or have your client fill out the complaint form that is on the US T's wesite.
