"Plan Payment Declaration" form

I just received a letter from Kathy Dockery, which makes mention of
"the Plan Payment Declaration form"
There's no such form in Best Case.
There's no such form on Ms. Dockery's web site of downloadable forms.
There's no such form in the USBC web site under either local forms or
court forms.
If the letter said "a Plan Payment Declaration form" I would just make
one up on pleading paper. However, the wording indicates use of a
specific form is required and there does not appear to be any such
form.
Can anyone help?
