LBR 1015-1 Consolidation

Assuming, under Balas and Morales case, they are spouses, the 1015-1 statement should say so (maybe that is what you mean about "Related case disclosure will be made").
The motion for joint administration is a noticed or emergency motion typically made on the first day of Chapter 11 cases. Here is a form, although it is probably overkill.
