You will need to prepare a fee app. It isn't too difficult. Check the local rules for what the fee app needs to include. You will need a declaration from the debtor regarding the services provided and the value.

It turns out that I (or someone at my firm) inappropriately filled out the attorney compensation portion of the petition as being paid in full and forgot to include the fees in the plan. How should I go about being paid in this case? It has never happened to me before.

Listmates-
I recently had a chapter 13 plan confirmed. The notice of intent to pay claims just came through, however I noticed that it did not include my attorney fees. It turns out that I (or someone at my firm) inappropriately filled out the attorney compensation portion of the petition as being paid in full and forgot to include the fees in the plan. How should I go about being paid in this case? It has never happened to me before.
Thanks!
