Motion to Voluntarily Dismiss Chapter 13 Case Converted from 7

Hi Listmates,
Does anyone have a sample Motion to voluntarily dismiss a Chapter 13 case that was converted from a 7? According to local bankruptcy rules, it must be a noticed motion with a hearing date.
