Send client a bill for the consultation (unless you told client 1st consultation was free), and bill for all of the calls. In the letter, including the bill, tell client retainer is used up (if so, but it sounds like it from your description), and that if client wants you to file the bankruptcy they have to come in with the remaining money in 30 days or you will close the file.
I tell the clients, and put it in my retainer, that I will not take calls for more than a year for the initial payment.
