New Ch 13 Order Form F 4003-2.4

It seems that the new order form F 4003 -2.4 applies only to debtor's primary residence ("ORDER ON MOTION TO AVOID JUNIOR LIEN ON PRINCIPAL RESIDENCE UNDER 11 U.S.C. 506(d"). Does anyone know if the same form can be used for avoidance of 2nd TD on a rental property perhaps by taking out the work "principal residence"?
