Getting atty fees from a cash-for-keys settlement of home retention

This client is currently in a chapter 13 case (pre-confirmation). The
adversary case against the mortgage lender settled for $5,000 cash-for-keys.
The bank counsel is preparing the settlement agreement. Client agreed that
from that $5,000, two thousand will go to pay my legal fees. The next step
for this client is chapter 7.
Do I need to give special instruction (regarding my $2,000 fee) to bank
counsel who is preparing the settlement agreement?
I understand that I need to submit a fee application. Could someone share a
sample from a similar situation?
Is there any other issue that I am forgetting?
