I checked w/Best Case b/c I thought they made a mistake. Although they're using an older form (which they are going to update), they said they took the language directly from the official form. I looked up the official form on the CDCA website and I found the language there too.
I've attached the Best Case form (note that some of our exclusions are directly contradictory to the language in Section 5) and the official CDCA form.
