Feel free to use it as a form.
If you are watching your daily email from ecf, you
will know which parties you can put in the "served
electronically" part of the proof of service.
Make sure you send in a copy to the court with all of
the envelopes and postage within two days (don't know
how that works when you file an order on Friday).
