Request for Sample "Statement and Request to Correct Case Chapter in

Hi all,
I inadvertently filed a chapter 7 case under chapter 13 in CM/ECF. The
clerk instructed me to file a *Statement and Request to Correct Case Chapter
in CM/ECF System. *Apparently there is no form for this. Does anyone have
a sample I can use? Not sure what information I need to include in it.
