Loan Modification Issue

A client for whom I just field a Ch13 case, without my knowledge, has been
talking to her lender and has just been approved for a "trial payment
program" in order to hopefully ultimately get a permanent loan
modification. Is there a form that I need in order to get Court approval to
get into the trial program? or the permanent loan modificatoin (assuming
that the debtor will get one at all)?
