Eastern District Request for Notice Form

Does anybody know if there is an equivalent ECF-NEF request for notice form for the Eastern District? I couldn't find one on their website or on my software programs. Or do I have to do one of those old fashioned requests for special notice that I haven't done in years?
If so, next request is does somebody have one they could email me?
