Relief from Stay on co-debtor

I just filed a MRS for a creditor in a Ch. 13 case. There is a co-debtor on title to the property (and on the loan). I forgot to ask for relief against the co-debtor.
How is this done on our required court-approved forms? Do I need to amend the MRS to add the co-debtor?
