Need Final Report under FRBP 1019(5)

Does anyone have a Final Report filed under FRBP 1019(5)(A) they could share just so I can use as a template of sorts? This is the report that gets filed upon conversion of a Ch. 11 case to a Ch. 7. It's been about 12 years since I've done one and I would just like to see one to refresh my memory.
