renew judgment

Is there a form for renewal of a judgment in Bankruptcy court? What
is the form number and how do I get it if there is one?
I obtained a judgment against a debtor in a section 523 action in
March 1995 and would like to renew the judgment.
Any assistance would be greatly appreciated.
