Language to Require Accurate Credit Reporting

I confirmed a Chapter 11 case about 4 years ago. Since then, the bank
servicing the mortgages that were modified under the Plan have refused to
report payments. I'm in the middle of settling and want to request that
they report the loans accurately. However, as I sit here to draft the
language, Im consulting NCLC's Fair Credit Reporting treatise, but I cannot
find the wording for how I should request the bank report. Has anybody out
there have to come up with such language or has some ideas on how to
request that the mortgage holder report accurately?
