Debtor selling house in month 60 of plan; plan is already paid off

Debtor has paid off her chapter 13 plan. It's month 60 and she even got a
refund from the Chapter 13 Trustee. She just signed a purchase and sale
agreement on her house. Since the case isn't yet discharged and closed, (1)
do I need to obtain court approval for the sale, and (2) does the payoff of
the lender have to go through the Chapter 13 trustee so she can collect her
