Stripping Lien- First time

I am filing an advesary proceeding to strip lien. I have no problems with the second deed of trust because just one person; however to establish the first lien holder's security, could I attach debtor's declaration and a recent statement? The note changed many hands but the final lender is CountryWide Home Loans.
Also, upon serving the summons and complaint, I am required to serve a notice required by FRBP 7026 and LBR 7026-1. Is there a special form or format for this notice?
