Thanks to Mark Jessee, I found the new forms anyway..They are at the top of
the new local rules page (instead of incorporated into the forms section,
for some reason).

Oh, and to make things even more challenging, this new Form isn't even
available from the Court's website. And neither is the new proof of service
we're supposed to use for judges, nor are a bunch of other new forms that
are listed in the new manual..

I was just preparing an Order and decided to take a look at the court manual
online and found that the service list requirements for LOUs have changed as
of January 2009.
