I think Keith has it right, and in these situations I show up and explain [with cc of the letter(s) I've sent, and I call and all of that.
But ... does the trustee not have an obligation to make reasonable efforts to communicate with counsel?

Call trustee's office, ask for trustee's administrator, in a 7. But,
in a 7, you can miss one 341a. Trustee will send you a continued
hearing date. Cannot miss 2nd hearing.

This is true, but I believe that it is important for reputable attys to still appear personally --unless excused before hand by the Ch7 Trustee or their administrator, to explain the situation of the debtor. Yes, you could attempt to rely on the possible get one free continuance but I would never recommend it.

Yes it is. But I believe that unless the trustee excuses you, you should be gracious enough to appear and personally inform the trustee of the latest situation.

Most trustees are accommodating for situations like this.

I filed a case for joint debtors, H&W, and the 341 is this Thursday. On
Sunday morning the wife emailed me and said her husband was admitted to the
hospital after a heart attack Sunday morning. I faxed a letter to the
trustee's office yesterday and left a couple voice mails. I left another
voice mail this morning. No response. On THursday should I go to the 341
myself and explain, or go with the wife? H is having surgery today and
won't be discharged by Thursday, and, obviously, the wife would rather not
deal with the 341 meeting right now. What should I do?

If you do not hear from the administrator for the Ch7 Trustee, then you should go to the hrg and explain the situation to the Ch7 Trustee and request a continuance. Also, if possible, it is best to provide the trustee with some sort of letter or note from a physician that explains the debtor's situation. Also, Trustees usually do not like to examine just 1 spouse at a time unless there is VERY special situations.
