In California - Central District, I believe it is a one or two page form (notice of conversion) without a motion, if being converted for the first time.

Does anyone have a sample motion to convert ch 13 to ch 11 based on exceeding the debt limit?
Can this be done via stip with ch 13 trustee or UST? Or can it be done with just a notice?
