Form to use to notify the credit bureaus. This should work. You might want to check the bureau's addresses to make sure they are the most current.

Go into Forms on the Court Website. Under "Other Forms" click on Amended Statement of Social Security Number. The form and an instructional PDF will come up. But it doesn't address the Credit Bureau problem.

You should also prepare a declaration of your client testifying regarding
their SSI and file that declaration with the court.
Letters need to go out to the credit bureaus to alert them.
