"record of interest" required by 521(c)

Does anyone have any idea what is supposed to be filed to satisfy
the Code requirement in 11 U.S.C. 521(c) for a "record of interest"
held by the debtor in a child's 530(b) educational IRA account? Is
this supposed to be an account statement showing the balance
(presumably with the child's name and account number redacted)? Or
is it supposed to be a Declaration of some sort? I've already put
all the relevant information in Schedule "B" so I can't for the life
of me figure out what the requirement is. Legislative history
doesn't say much either.
