Mark, we never have a problem when this happens, but neither I nor anyone I've spoken with can pin a Code section or a reported decision on it. Seems as if it's a rule of logic that either a Plan should be modified or the case gets dismissed and the debtor re-files.
Seems like a, "call your trustee to get his or her take on it," sort of thing.
