Looking for sample "mixed" retainer agreement (hourly, then contingency)

This is for an attorney malpractice case. I don't want to do it fully on
contingency. Does anyone have a mixed retainer agreement sample where you
bill hourly up to a certain point, then it goes to contingency fee? Please
