Ch. 13 statement under 521(f)(4)

For those of you who actually file the required annual post
confirmation income/expense statements in Ch. 13 cases, what form(s)
do you use, and what ECF Event Code do you use?
We used to have a form which is no longer available for that.  Now I
tried using the new Schedule "I" and "J" and ticking the box that
says "A supplement showing post petition chapter 13 income...".  I
tried to file that using the event code "Statement", because it is
the statement required under 521(f)(4).  Well, ECF rejected that.  I
called and they said the pdf has to match the event code.  When I
explained there is no event code for what I'm doing, they said "You
have to use the event code for amending Schedule "I" and "J""  to
which I responded, "I'm not amending I & J".  This went on for about
15 minutes, whereon I was told that a supervisor would be calling me
back.
Seems like a lot of work for this.  Does anyone have the workaround?
I guess I should just slap a cover sheet on that says "Statement
Under 521(f)(4)" and maybe that will appease them...
