Catherine,
I try to do my employment applications for accountants in smaller cases
where the court is asked to approve a flat fee for a particular job with no
fee applications unless the requested fees exceed that approved by the
court. However, I have also filed fee applications for accountants.
I will send you a few samples to your direct email.

Drafting an Interim Fee Application for an Accountant -
1. Do you need one? All of the dockets on the cases I have found on pacer only hire the accountants, no one seems to file an application for the accountants to be paid.
2. Assuming you do need a fee application - I am guessing I would use the form application from the court's website and attach a Memo of P&A's which breaks out the categories and lists the benefit to the estate and the average hourly fee for the services performed in each category - the same as for a fee application for general counsel
3. Under case law and at the insistence of the UST's office, general counsel must apply for fees pursuant to 330. Is that the same for accountants? or are they under 331. The application to employ refers to both 331 and 330.
4. Or do accountants bill the debtor on a monthly basis and the debtor pays the accountant and the accountant submits Professional Fee Statements to the UST? The application to employ states that the Accountant will submit to the court for approval of fees and costs and then ask the debtor for payment when the court approves the fees and costs.
Thanks in advance.
